EUTF OPEN ENROLLMENT
Good morning,
We ask your assistance in forwarding to your EUTF eligible employees who may have limited access to QUILL or a computer.
Thank you...
To All EUTF Eligible Employees:
As announced earlier, the Hawai‘i Employer-Union Health Benefits Trust Fund (EUTF) is having its Open Enrollment period during the month of November, from
November 2, 2009 through November 30, 2009. Employees will have the opportunity to make changes to their health care plans, for coverage to be effective January 1, 2010 through June 30, 2010. The EUTF has established a web site specifically for Open Enrollment: www.hieutf.org Here you will find information on the different health care plans that are being offered, the reference guide, the benefits fairs schedule, the October 2009 EUTF newsletter, and links to the various carriers.
Your personal Open Enrollment Notice is expected to arrive next week. We will make every effort to provide you your copy as soon as possible. The Reference Guide will be available in mid-November. The guide explains the open enrollment instructions and provides summaries of the different health care plans. For your convenience, the Reference Guide is available for viewing at: http://www.hieutf.org/wp-content/uploads/2009/10/active_reference_guide_2010.pdf
New plan offered this year: The EUTF is offering a new PPO 80/20 health care plan that will be administered by the Hawai‘i Medical Services Association (HMSA). The current 90/10 PPO Plan will be administered by the Health Management Associates (HMA). HMSA will NOT be offering a 90/10 PPO Plan this coming contract period. Employees wishing to retain a 90/10 PPO Plan will NOT need to complete an EC-1 Enrollment Form. The EUTF will administratively make the change for you and your medical plan will be administered by the HMA. Employees wishing to retain a HMSA PPO Plan must complete an EC-1 form to enroll in the 80/20 PPO benefit plan. If no EC-1 form is submitted, and you are currently enrolled in the HMSA PPO plan, you will be automatically enrolled in the HMA 90/10 Plan. NOTE: “90/10” or “80/20” refers to most benefits; see Reference Guide for Active Employees for details.
Any changes to the health care plans must be made via the EC-1 form and submitted to your personnel representative by November 30, 2009 (firm deadline).
Form EC-1 (fillable): http://www.eutf.hawaii.gov/OE_2010/Form_EC-1__final_FORM_Oct09_.pdf
Form EC-1 Instructions: http://www.eutf.hawaii.gov/OE_2010/Form_EC-1__final_INSTRUCTIONS_Oct09_.pdf
Confirmation notices will be generated for employees who make changes to their health plan enrollment.
As a reminder, the EUTF Open Enrollment informational sessions will be held at the UH Mānoa campus on Tuesday, November 3, 2009 and on Wednesday,
November 25, 2009, at Kuykendall Auditorium. There will be three sessions held each day: 8:30 a.m., 10:30 a.m. and 1:30 p.m. Other sessions will be held throughout the islands. See schedule: http://www.hieutf.org/?page_id=74 The Governor has authorized up to two (2) hours (including travel time) to attend an informational session during work time. Employees are encouraged to attend a session due to the changes to the health care plans. Representatives from the health plan carriers will be present and available to answer your questions.
Thank you.
